A Clinical Study.of Gastric Lymphoma and Diagnostic Significance of Endoscopy


Abstract
A total of 17 cases of gastric lymphoma admitted to the Dongsan Medical Center, Keimyung University from 1972 to 1990 were reviewed.
1. The data on age and sex distribution shows that gastric lymphoma is seen more commonly in men (male to-female ratio 24: 1) and peak incidence occurred in the 6th decade of life.
2. Most common presenting symptoms were abdominal discomfort and pain (58.8%). Abdominal mass was observed in 5 cases (29.4%), hematemesis and melena in 1 case (5.9%) respectively. The median duration of symptoms is 9.18 "weeks and range from 1 week to 9 months.
3. Histologic and stage classification were as follows : Most of cases were non-Hodgkin¢¥s lymphomas (16 cases). According to Rappaport and Rye classification, all of NHL were diffuse type-(12 cases of histiocytic type, 3 of mixed lymphocytic and 1 of poorly differentiated type) and 1 case of HD was mixed cellularity type.
4. Using Ann Arbor staging system, stage I in 2 cases (11.8%), stage II in 3 cases (17.6%), stage III in 2 cases (11.8%) and stage IV in 10 cases (58.8%) were observed.
5. Diagnosis of lymphoma was achieved by operation in 7 cases, node biopsy in 7 cases, and endoscopic biopsy in 3 cases.
6. Endoscopy established diagnosis of cancer in 4 cases, benign ulcer in 1 case and suspecting lymphoma in 7 cases (50%). Specimens from endoscopic biopsies were obtained in 12 patients and the result was as follows Lymphoma, 3 ; suggested lymphoma, 2 ;atypical lymphoid cell infiltration, 2 ;stomach cancer, 2 ;ulcer, 1 ;false negative,` 2.
7. Sites of involvement were confirmed by endoscopy, UGI, Mand operation. Tumor involved most frequently distal portion and duodenal involvement observed in 4 cases. 8. Treatment was performed as follows : Operation only in 1 case, combination chemotherapy only in 9 cases, operation with chemotherapy in 5 cases, and operation with chemoradiotherapy in 2 cases.
